A Message from our Nurse:
Postural (back) and Gait Screening
George Read Middle School will be conducting a screening program to find children who may have a curvature of the spine. With early detection, through these screening programs, spine deformity can be prevented. The screening is carried out by the school nurse and physical education teachers, and takes less than 30 seconds per child. The examiner looks at the child's back as she stands and bends forward. You will be notified if a possible curvature is found.
The nurse’s office is open from 7:15 am to 2:45 pm daily. Parents are welcome to stop in or call during these times.
Students may come only with a signed pass from their teacher to the nurse’s office.
Students with casts, splints, arm slings, ace bandage wraps, stitches, or on crutches must report to nurse upon return to school with a Doctor’s note stating the problem, instructions and any restrictions
Student’s emergency cards are to be filled out on the front and back and signed in 2 places in front.
Make sure to mark the Tylenol section for it to be given.
These cards are important to the nurse and administration and are to be returned promptly to the nurse.
If there any address or phone number changes during the school year, please send them in to the nurse or secretary.
Please write down all names and phone numbers for someone other than parent/guardian who can be called.
According to Delaware laws and Department of Education regulations, all children entering public school for the first time are required to have writtenproof from the doctor’s office of the following:
5 or more doses DtaP, DTP, or TD vaccine
4 doses of IPV or OPV (Polio) vaccine
2 doses of Measles vaccine
1 dose of Rubella vaccine
1 dose of Mumps vaccine
OR 2 doses MMR.
3 doses of Hepatitis B vaccine (must have before entering 7 th grade)
1 dose Varicella-recommended
Tuberculosis (results of Mantoux screening (PPD) completed within the past 12 months.
Please send proof from the doctor’s office any additional immunizations the student may receive during the summer or school year.
Students who are entering public schools for the first time in Delaware must have a physical examination completed by a licensed physician, nurse practitioner or a physician’s assistant.
Students who would like to participate must have a recent physical examination on file before they may try out for an athletic team. The physical may be from their personal doctor OR they may have a sports physical completed in school in September on a designated day.
The school physical is a one-time occurrence early in September. The student must return the parent/guardian's permission slip to have the physical. The cost is $17.00, cash or a check made out to First State Orthopedics.
The nurse will dispense all medication taken by students during the school day. Medication and directions for its administration must be taken to the nurse’s office at the beginning of the school day.
The prescription and the medication shall be current and long-term prescriptions shall be re-authorized at least once a year. Prescription medication must be in its original container and any over-the-counter medication or non-prescription medication must also be in its original container. Both must be accompanied with a signed permission slip from the parent/guardian. Under no circumstances may a student carry or take medications by themselves.
When additional medication is needed, the empty container will be sent home with a medication return slip to be signed by the parent. This slip and filled container is to be returned to school the following day.
