REGISTRATION REQUIREMENTS

• Recent utility bill (within 60 days)
• Original sales or lease agreement (must include name, address, and signature of both the tenant/landlord or buyer/lending company)
• Colonial School District "Residence Verification Form" *

*If the family is living with someone, they must pick up a RESIDENCE VERIFICATION FORM and have it notarized. (a copy of one of the above "proof of residence " must also be included)

Identification -

The parent or guardian must provide photo identification. A valid Delaware driver's license or state identification card is preferable, but not required.

Custody Arrangements (if applicable) -

If the student is not living with parent(s), proof of legal guardianship/custody is required.

• Social Services Placement Letter (original)

Birth Certificate -

Official State Birth Certificate of the student is required. Hospital Birth Record is not acceptable.

• State Certificate of Live Birth
• If born in Delaware, you can contact the NCC Office of Vital Statistics to request a copy if needed.

• Letter, physical exam form, other documentation from physician
• Tests that are required: Mantoux, TB test, Hepatitis B proof of completed 3 dose series

Students who are entering Kindergarten, coming from a private school, or from out of state are required to have a physical examination.

Copy of last report card or withdrawal form.

• If at all possible, please provide as much educational documentation as possible, especially for students with special needs.
